You have two good options. You can get on CT-15 S from Park St and CT-106 S/Old Stamford Rd and take that to exit 28, which is Round Hill Rd. Take Lake Ave to W Elm St.
Or Take CT-106 S/Old Stamford Rd, Lapham Rd, Hollow Tree Ridge Rd and Middlesex Rd to I-95 S in Darien, then from there follow I-95 S to Arch St in Greenwich. Follow Arch St, Sound View Dr., and Field Point Rd to W Elm St.

Our patients find it much more convenient to fill this out at home at their convenience, rather than trying to do so at our office when they may not have all the information available..
You will be asked to fill out a general questionnaire regarding your health and reason for consult. Shortly thereafter the doctor will bring you to the exam suite where you will discuss your medical history with the doctor and the examination will begin. If a biopsy is necessary, it usually can be done at the same time. Medications,if needed, will be prescribed and a follow up appointment will be made.
