What Is a Biostimulator?

Many people are familiar with traditional dermal fillers--injectables that add immediate volume to areas like the lips or cheeks. Biostimulators work differently, and in many ways, more profoundly. Rather than simply filling a space, they prompt your skin to regenerate its own structural proteins, primarily collagen and elastin, the two building blocks responsible for firm, resilient, youthful-looking skin.

Think of it this way: a traditional filler is like placing a cushion under the skin. A biostimulator is like instructing the body to reupholster itself from scratch.

The results develop gradually over several weeks to months, which is exactly the point. Because the improvement is driven by your own biology, outcomes look and feel completely natural; not filled or frozen, but genuinely refreshed.

What Is a Biostimulator?

Why Collagen Matters, And Why We Lose It

Starting in our mid-twenties, the body produces approximately 1% less collagen each year. By the time visible aging becomes noticeable–sagging contours, deepening folds, crepe-like skin texture–significant collagen loss has already accumulated beneath the surface. Sun exposure, stress, smoking, and hormonal shifts (particularly eeaaround menopause) all accelerate this process.

Topical skincare can support collagen at the surface level, but no cream or serum can meaningfully replace structural collagen in the deeper dermis. That’s where biostimulators make their most compelling case.

RADIESSE®

RADIESSE is one of the most established biostimulators available today. Its active ingredient is calcium hydroxylapatite (CaHA), a mineral compound that naturally occurs in human bone. When injected, the CaHA microspheres create an immediate scaffolding effect while simultaneously triggering a sustained collagen-building response in the surrounding tissue. Over time, the microspheres are safely absorbed by the body, leaving behind a network of newly formed collagen.

RADIESSE is particularly effective for:

RADIESSE also comes in a diluted formulation known as RADIESSE (+) Hyperdilute, which spreads across a wider surface area to treat overall skin quality rather than targeted volume. This makes it an excellent option for patients experiencing diffuse skin laxity rather than discrete hollowing.

Results from RADIESSE typically become visible within four to six weeks and can last 12 to 18 months or longer, depending on the treatment area and individual metabolism.

SCULPTRA®

Sculptra takes a different molecular approach. Its active ingredient is poly-L-lactic acid (PLLA), a synthetic but fully biocompatible material that has been used in dissolvable sutures for decades. When introduced into the deep dermis, PLLA microparticles act as a slow-release collagen stimulus, gradually activating fibroblasts (the cells responsible for collagen production) over a series of months.

Unlike most injectables, Sculptra is intentionally gradual. The treatment is typically administered across two to three sessions spaced four to six weeks apart, allowing the collagen response to build progressively. This phased approach is what makes the outcome so natural-looking, there is no single dramatic shift, just a slow, steady improvement that people around you will notice without being able to pinpoint why.

Sculptra works best for:
- Widespread volume loss in the mid-face, temples, and cheeks
- Improving overall skin thickness and elasticity
- Addressing the hollow, "sunken" appearance that can develop with age or significant weight loss
- Patients who prefer a gradual, maintenance-oriented approach

Results from a full Sculptra course can last up to two years, making it one of the longest-lasting injectable treatments available.

Since results are self-generated, they move and age with your face Biostimulators are not ideal for patients seeking immediate volume correction or for those with certain autoimmune conditions. Both RADIESSE and Sculptra are administered by injection in an office setting, typically under topical anesthetic. Sessions are generally well-tolerated, with mild swelling, bruising, or tenderness in the days that follow. There is no significant downtime, though we typically recommend avoiding strenuous exercise for 24 hours and staying out of direct sun for a few days post-treatment. For Sculptra specifically, patients are instructed to massage the treated areas for five minutes, five times a day, for five days following each session. This is a simple but important step in achieving even results.
